The human papillomavirus vaccine and risk of anaphylaxis.

چکیده
509 I n this issue of CMAJ, Brotherton and colleagues 1 report a comprehensive investigation revealing higher than expected rates of apparent anaphylaxis following vaccination with the quadrivalent human papillomavirus (HPV) vaccine in Australian children. The cause of these reactions remains somewhat unclear and needs further investigation. Of note, rates of anaphylaxis, if confirmed, may not be as high in other populations. Further investigations may assist in clarifying differences between the Australian study and other reports. The use of a review panel with broad expertise in vaccines , allergy, immunology and pediatrics by Brotherton and colleagues and their use of the Brighton collaboration definition of anaphylaxis strengthen the evidence that the reactions were indeed anaphylaxis. This also strengthens their conclusions that the rate of anaphylaxis in the Australian population following HPV vaccination was higher than the rates observed following other vaccinations. Before concluding that the HPV vaccine is associated with higher rates of anaphylaxis than other vaccines everywhere, cases in other populations should be reviewed. In the United States, 15 cases of anaphylaxis or anaphlyactoid reactions following HPV vaccination were reported to the Vaccine Adverse Events Reporting System in 2007. As of July 21, 2008, 11 cases have been reported in 2008. Over 13 million doses of this vaccine had been distributed as of the end of 2007. 2 Although there may be underreporting, the rate of about one case per one million vaccinations is consistent with the rate of anaphylaxis following several other vaccines. Regardless of the true rate, the causes of rare serious adverse events should be identified and vaccines made as safe as possible to maximize benefits and maintain public confidence in vaccines and immunization programs. Differentiating hypersensitivity reactions from fainting and anxiety reactions can be difficult, especially in busy, school-based clinics. The Clinical Immunization Safety Assessment Network, sponsored by the US Centers for Disease Control and Prevention, has developed guidelines and an algorithm to assist clinicians in the assessment of suspected immediate hy-persensitivity reactions and decision-making about administering subsequent doses of vaccine. 3 Anaphylaxis that occurs within minutes of exposure is usually associated with pre-existing IgE antibodies induced by prior exposure to the allergen. Brotherton and colleagues speculate that the reactions observed could have been due to prior sensitization from yeast proteins in the hepatitis B vaccine or HPV antigens from prior infections. However, 5 of the 7 women developed anaphylaxis after the first dose of …

Construction a DNA Vaccine Containing Human Papillomavirus Type 16 Early Genes as a Potential Vaccine for Cervical Cancer Prevention and Therapy
Background and Objectives: Some of the human papillomaviruses (HPVs) can infect genital tracts and are sometimes associated with anogenital tract cancers. HPVs induced cervical cancers through the expression of E6 and E7 genes by inactivating the tumor suppressor proteins. In this study, E6 and E7 genes were chosen in order to construct an expression vector which is able to express ...
متن کاملCTL Responses to a DNA Vaccine Encod-ing E7 Gene of Human Papillomavirus Type 16 from an Iranian Isolate
Background: Cervical cancer is the most prevalent tumor in developing countries and the second most frequent cancer among female population worldwide. Specific human papillomaviruses and, most notably, HPV types 16 and 18 are recognized as being caus-ally associated with cervical carcinomas. The early HPV type 16 genes, E6 and E7, di-rectly participate in the in vitro transformation of primary ...
متن کاملAnaphylaxis following quadrivalent human papillomavirus vaccination.
BACKGROUND In 2007, Australia implemented the National human papillomavirus (HPV) Vaccination Program, which provides quadrivalent HPV vaccine free to all women aged 12-26 years. Following notification of 7 presumptive cases of anaphylaxis in the state of New South Wales, Australia, we verified cases and compared the incidence of anaphylaxis following HPV vaccination to other vaccines in compar...
متن کاملAssociation between Human Papillomavirus Infection and Risk of Prostate Cancer
Human papillomavirus (HPV) is one of the most common causes of sexually transmitted diseases worldwide and is the etiological agent of cervical and other anogenital malignancies. Since HPVs have been shown to possess oncogenic potential, an association between HPV infection and prostatic cancer (PCa) has been suggested. There are conflicting reports on the impact of HPV infection on the deve...
متن کاملHuman Papillomavirus Type16- L1 VLP Production in Insect Cells
Objective(s): Infection by high-risk papillomavirus is regarded as the major risk factor in the development of cervical cancer. Recombinant DNA technology allows expression of the L1 major capsid protein of HPV in different expression systems, which has intrinsic capacity to self-assemble into viral-like particles (VLP). VLPS are non-infectious, highly immunogenic and can elicit neutralizing...
متن کاملPreparation and in vivo anti-tumor evaluation of human papillomavirus E7 adjuvanted with Montanide ISA 266 as a vaccine candidate
Introduction: Human papillomavirus (HPV) 16 E7 protein is expressed constitutively by HPV-infected tumor cells. Mutant versions of E7 are considered as safer candidates for immunotherapy of cervical cancer. Different strategies including formulation with adjuvants are used to induce a potent immune response against antigenic proteins. Methods: In this experimental study, we used Escherichia co...
